Background

Verbal consent is acceptable for a client's Release of Information record until such a time that you are able to get a signed consent form from the client. In HMIS there is an option under Documentation Type for Verbal Consent. A report has been developed for agencies to see those clients with verbal consent so they can go back and get a signed consent form when it's possible.

 

Instructions

Verbal consent is NOT a permanent replacement for a completed Consent To Share Protected Personal Information form. Please only select Verbal Consent from the Documentation dropdown on the Release of Information page if you are not able to obtain the client's consent to share their protected personal information in HMIS any other way:

 

 

Finding Clients with Verbal Consent ROI Records

To see which clients must be contacted to complete a Consent To Share Protected Personal Information form (electronically in HMIS or on the paper form):

 

1. Click the menu button

2. Click "Reports"

3. Click "Data Analysis"

4. Click "Orange County Clarity System Reports"

5. Click "Run" next to "Clients with Verbal Consent ROI Records"

 

 

6. Click "Filters"

7. Enter your agency's name

8. Click "Run"

 

You can click the number that appears in the client's "Personal ID" column to see a link to the client's Profile in Clarity.
