Background

The following HMIS Skills Test is intended to test HMIS Users' ability to apply the information about HMIS functionality learned through OC HMIS Online Training Courses HMIS Part 1 and HMIS Part 2.

 

All new HMIS Users are required to complete the HMIS Skills Test in order to gain access to HMIS, and existing HMIS Users will be required to complete the HMIS Skills Test on an annual basis to maintain their HMIS accounts.

 

To inform the HMIS Help Desk of completed HMIS Skills Tests, please submit the HMIS Account Update & Testing Form.

HMIS Skills Test Procedure

Complete the tasks below, in the order they appear, on the Clarity training site. You may need to refer back to the HMIS Part 1 and HMIS Part 2 training courses to complete the HMIS Skills Test. Please complete the HMIS Skills Test on your own, without working with others.

 

Your Agency Administrator can provide you with login information for the Clarity training site. If your Agency Administrator does not have login information for the Clarity training site, they will need to submit a ticket with the HMIS Help Desk. 


When entering client information for this exercise, you may enter any answers and fictional information you like unless otherwise specified. Some tasks require you to select certain options, for example, task 2 specifies that Client 1's Release of Information record must be "Attached PDF."

 

When you are finished, you will need to provide the following information to your Agency Administrator. Please make note of the following as you complete the HMIS Skills Test:

  • Username you used in the Clarity Training Site:
  • Client 1 Unique ID and Fake Name
  • Client 2 Unique ID and Fake Name
  • Client 3 Unique ID and Fake Name
  • Project 1 Name
  • Project 2 Name
  • Date on which the test was completed
  • PDF Attachment of the Program Roster Report

 

HMIS Skills Test

  1. Add a client record for Client 1, and complete the Release of Information.
    • Client 1 should be at least 18 years old.
    • The Document Type for the Release of Information should be Attached PDF.
    • The PDF document itself can be any PDF document.
  2. Add another client record, and complete the Release of Information for Client 2.
    • Client 2 should be a minor and will be the child of Client 1. 
    • The Document Type for the Release of Information should be Household. 
  3. Add Client 2 to Client 1’s Global Household.
    • Ensure that Client 1 is the head of household. 
  4. Add a public alert to Client 1. 
    • In the alert, describe a concern or situation that would be helpful for other agencies working with the client to know about.
  5. Enroll the household in any project in the OC Training Agency. This will be Project 1.
    • Set the Project Start Date in Project 1 for both clients to be one month prior to today (For example, if today is 08/01/2020 the Project Start Date should be 07/01/2020). 
    • While the data you use will be fake, please ensure that your responses to the enrollment questions are consistent with the presumed parent/child relationship between Client 1 and Client 2. 
    • Pretend that Client 1 and Client 2 spent the night before in an Emergency Shelter. 
  6. Add services to Client 1's enrollment, and add a note to the service.
    • Use any of the services in the project.
  7. Complete a Status Assessment for Client 1 and Client 2. 
  8. Complete an Annual Assessment for Client 1 and Client 2.
  9. Add project level client notes to Client 1 and Client 2's enrollments.
    • For both notes, enter the time spent on the case management session in the Time Tracking field.
  10. Add a private note to Client 1’s enrollment.
  11. Add a public note to Client 1’s enrollment.
  12. Using the Attendance tab, add the Food: Meal service to Client 1 for 7 days. 
  13. Create a new client profile for Client 3, and enter a Release of Information.
    • Client 3 should be at least 18 years old.
    • The Document Type for the Release of Information should be Verbal Consent. 
  14. Add Client 3 to the global household of Clients 1 and 2. 
  15. Enroll Client 3 into Project 1 with Clients 1 and 2.
    • Client 3's Project Start Date should be 7 days ago. 
  16. Exit all 3 clients from Project 1.
    • The Project Exit Date should be two days ago.
  17. Enroll all three clients into a new project, Project 2, that is different than the previous enrollment they have.
    • The Project Start Date should be yesterday’s date.
  18. Exit Client 3 from the project.
    • Client 1 and Client 2 should remain enrolled in the project.
    • The Project Exit Date for Client 3 should be today’s date.
  19. Run the Program Roster Report for either of the projects that the three clients were enrolled in and download the PDF.
    • The reporting range should be for the past month, ending today. The status should be Active within Report Date Range. 
  20. Give your Agency Administrator the following information:
    1. Client 1 Unique ID and Fake Name
    2. Client 2 Unique ID and Fake Name
    3. Client 3 Unique ID and Fake Name
    4. Project 1 Name
    5. Project 2 Name
    6. Date on which the test was completed
    7. PDF Attachment of the Program Roster Report

FAQ

Where do I take the HMIS Skills Test?

Make sure that you take the Skills Test on the OC HMIS Training site. Do not use the live OC HMIS site.

 

Do I need to take the HMIS Skills Test?

There are two situations which require completion of the HMIS Skills Test:

  1. If your account was created before April 1, 2020 - You will need to take the HMIS Skills Test before November 20th, 2020, in order to complete your required 2020 HMIS User Recertification.

  2. If you are a new HMIS User - You will need to complete the HMIS Skills Test in order to receive your HMIS account login information.

 

Why can't I log in to the OC HMIS Training site?

For the HMIS Skills Test, you should use the agency's training account to login, not your personal account login. Please contact your Agency Administrator regarding your agency’s training account login information.

I am an agency administrator, but I don’t know what my agency's credentials are for the training site.

Please submit a ticket to the HMIS Help Desk requesting training account information.

 

Do I have to complete the HMIS Skills Test in one session once I begin?

You can complete the HMIS Skills Test at whatever pace is comfortable for you before your Agency Administrator submits your completed HMIS Skills Test information. Please note that you need to complete the HMIS Skills Test before your HMIS account can be created or unlocked.

 

What do I do if I do not receive a 100% on the test?

You do not need to receive 100% on the test to receive a new HMIS account or to count as completion of the 2020 HMIS User Re-Certification process. If you score less than 100% in any section of the test, please review the OC HMIS Knowledge Base articles that are highlighted in your test results. Check in regularly with your Agency Administrator so they can review your HMIS data quality.
